Frailty Care Coordinator

Frailty Care co-ordinators help patients over the age of 65 with a degree of Frailty navigate the health care system and aid them to make the right connections, with the right teams at the right time. They advocate for patients and promote shared decision making to help patients be more active in their healthcare journey. Care co-ordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs.
